Thank-you for conducting home blood pressure monitoring as requested. Please monitor and record your blood pressure at home for 7 consecutive days.
Monitor your blood pressure on two occasions in the day- in the morning (between 6am and 12noon) and again in the evening (between 6pm and midnight).
Please sit still for at least 5 minutes before measuring your blood pressure. Please remain silent during the measurement. Avoid caffeine or smoking for 30 minutes prior to the measurement as they can temporarily cause a rise in your blood pressure.
On each occasion please take a minimum of two blood pressure readings, leaving at least a minute between each. Record the lower reading in the diary.
Use the table on the next page to record your blood pressure readings. The numbers you write down should be the same as those that appear on the monitor screen. Please do not round the numbers up or down.
Once you have recorded a total of 14 blood pressure recordings please calculate the average blood pressure value as detailed on the diary overleaf.
